A single flat loop from Via dei Fori Imperiali to the Circo Massimo. It heads south past the Circo Massimo and the Baths of Caracalla to the Basilica di San Paolo loop at km 4 to 7, crosses the Tiber twice through Marconi and Testaccio, follows the Lungotevere north past the Isola Tiberina and crosses the Ponte Vittorio Emanuele II to the Via della Conciliazione spur at St Peter's around km 13 to 14. Ponte Cavour and Ponte Regina Margherita lead into Prati and the Trionfale and Delle Vittorie streets, then the Foro Italico out-and-back at km 25 to 28 and the Ponte della Musica into the Flaminio and Villaggio Olimpico loop at km 30 to 34. The Lungotevere returns through the Ripetta underpass to Piazza del Popolo, and the last seven kilometres run Via del Babuino, Piazza di Spagna, Via del Corso, Corso del Rinascimento and Piazza Venezia to the finish. The organizer gives 8 to 31 m of elevation and about six kilometres of cobbles, concentrated in the historic centre.

Three decisions to make before race day
Choose your entry category before paying
If you live abroad, you race competitively only with a RUNCARD and a medical certificate (including rest and exercise ECG and spirometry) uploaded ten days before, or non-competitively, timed but unplaced. The paperwork takes weeks.
More detail
A runner living abroad can enter competitively only with a RUNCARD plus a medical certificate covering an examination, urinalysis, rest and exercise ECG and spirometry, uploaded to the entry platform ten days before the race, or non-competitively with a time but no placing by accepting the disclaimer. The paperwork, not the fee, is what takes weeks.
Declare an honest personal best by 17 February 2027
Your declared best, due by 17 February 2027, sets your start grid. In 2026 the last wave left fifty minutes after the first while gates were timed from the first wave, so back-grid runners had less margin. 2027 grids and gates are still to come.
More detail
The declared best fixes your start grid, and in 2026 the last wave left fifty minutes after the first while gate clocks were fixed local times computed from the first wave. A back-grid runner therefore had less margin at every gate than the headline limit suggested; the 2027 grid scheme and gates are still to be published.
Plan the last seven kilometres, not the first
About 6 km of cobbles, concentrated in the historic centre at the end: choose shoes and pacing for sampietrini at km 36 to 41, not for the flat Lungotevere.
More detail
The organizer states about six kilometres of cobbles on a course whose closing section runs Piazza del Popolo, Via del Babuino, Piazza di Spagna, Via del Corso and Piazza Venezia before the Circo Massimo. Choose shoes and pacing for sampietrini at km 36 to 41 rather than for the flat Lungotevere kilometres.
